How Coverage Verification Works

Our staff can review the information shown on your current insurance card and help determine whether the clinic participates with your plan. Verification is not a guarantee that an insurer will pay every charge. Your plan determines eligibility, deductibles, copayments, coinsurance, exclusions, and authorization requirements.

What to Bring

• Your current insurance or Medicaid card
• A government-issued photo ID when available
• The subscriber’s name, date of birth, and other information requested by the plan
• The patient’s current contact information

Before Your Visit
